Job Description:
Position Overview:
The Treatment Support Coordinator (TSC) will play a vital role in ensuring the continuum of care for individuals diagnosed through Active Case Finding (ACF) activities. The TSC will be responsible for active participation during ACF activities as per duty assigned, facilitating sample transportation, timely registration and treatment initiation, promoting adherence, conducting contact screening, and ensuring follow-up until treatment completion. The role involves close coordination with ACF teams, diagnosed individuals, health facility staff, and other stakeholders to ensure high-quality care, accurate documentation, and effective reporting.
Key Responsibilities:
- Ensure timely and complete registration of diagnosed TB and lung health patients at designated health facilities.
- Facilitate prompt treatment initiation and provide regular follow-up support to enhance adherence.
- Educate patients and their families on treatment regimens, adherence, side effects, and the importance of completion.
- Collaborate with healthcare providers, treatment supporters, and district-level stakeholders to ensure coordinated patient care.
- Monitor treatment progress, document outcomes, and report regularly to the provincial coordinator.
- Conduct community-based visits for contact tracing, treatment adherence support, and patient follow-up.
- Maintain accurate records and ensure timely data entry in digital platforms (MIS/DHIS2).
- Support the timely sharing of all relevant data—from diagnosis to enrolment and treatment completion—for onward transmission to the head office as required.
- Perform any other duties assigned by the supervisor related to program implementation.
Required Qualifications and Skills:
- Bachelor's degree in a health-related or social science field.
- Minimum 1–2 years of relevant experience in healthcare service delivery or public health programs.
- Basic understanding of TB case management and treatment protocols; knowledge of other lung diseases such as COPD, Asthma, Pneumonia, etc., is an advantage.
- Strong interpersonal and communication skills; ability to build rapport with patients and health providers.
- Demonstrated ability to work effectively in team settings and under minimal supervision.
- Willingness to travel to remote and hard-to-reach areas.
- Basic computer literacy and familiarity with digital health reporting systems (e.g., DHIS2).
- Possession of a valid motorcycle driving license and ownership of a motorcycle (travel expenses will be reimbursed as per organizational policy).

- Percentage of diagnosed individuals registered at facilities within defined timelines
- Rate of timely treatment initiation among registered individuals
- Treatment adherence rate and successful treatment outcomes
- Number of education sessions conducted with registered individuals and their families
- Accuracy and timeliness of data entry and reporting
